Stage 8 (Canyon) in UN Squadron (SNES) on Gamer mode. This level is hard enough as it is, but what is worse is the boss actually bugged up. Not only is it extremely rare for him to stay on screen 100% of the time, but his bombs actually started shooting two sets of missiles about halfway. Lucky to get through this mayhem with only 2 hits.
