Effective ways to improve mental health exist, both treatments for individuals and broader approaches encompassing public health, services, and policies. However, these are often not available to everyone, even in high-income settings, and a large treatment gap exists in many countries. Implementation research seeks to understand which interventions work in real-world settings and to identify barriers to delivering these interventions at scale, as well as ways to overcome these barriers. Too often, research produces both interventions and implementation strategies (e.g., financing, facilitation, training) that are difficult to scale due to misalignment with the political, cultural, policy, system, community, provider, and individual realities of real-world settings. The Lancet Psychiatry Commission on Transforming Mental Health Implementation Research considered strategies for transforming research to produce more actionable evidence to help narrow the mental health implementation gap.

Enhanced transdisciplinary collaboration among researchers, implementers, and intended intervention participants, including people with lived experience of mental illness, across all stages of mental health intervention and implementation research is central to the Commission’s recommendations. We need to replace the traditional translational research paradigm guiding most mental health implementation research with an integrated approach. The recommendations for transforming mental health implementation research originated in considerations for improving mental health implementation research in the high-income context and are driven by research from high-income countries but the Commission includes successful examples from low-income and middle-income countries and calls for expansion of implementation research in these settings.
