This was the only map I had success with: Belden. Here are the steps I took:
- Open the hero gates as soon as possible (even from a distance), so heroes can level quicker for more soul energy.
- I try to maintain my level at -3.
- I try to reach prestige 300, 500, 800, 1200, 1500 as soon as possible for the torture gimmicks.
- I use the smallest jail cell and avoid the lantern gimmick, to keep as much energy for the torture devices.

I played through the campaign on Medium and decided to give a try on Hard after reading about it on the Steam forums. Surprisingly easier now, with all the knowledge and skills I gained from finishing the game.
The main challenge here was to prevent the fuel tank from exploding. Again, surprising easy after discovering that the fuel tank hotspots are on the tank itself, the ground flames are merely cosmetic; I confirmed that with the thermal view tool and the ground was completely cool.
Everything else is just cleanup.
