The entrance to the Fadefields Vault is located in the Dissected Plateau, on an elevated area across the river from Idolator's Noose. To reach this area, approach from the northwest near the river. Use your grapple to cross The Stubs and find the Arch of Inceptus at the top.

Use all three Vault Key Fragments here to open the vault and challenge the Primordial Guardian Inceptus.
