*Spoilers*

Ending 12 from The Binding of Isaac.
This is confirmed to be the true, inevitably absolutely final ending.

You know, not the one that you get after Isaac beats Mom. Or Mom's Heart. Or Satan.
The one you get after Isaac beats... Isaac.

(Edmund himself called it the "true final ending" in an interview:)
http://gamasutra.com/view/news/39907/McMillen_details_The_Wrath_Of_The_Lamb_as_Binding_Of_Isaac_sells_450k.php

It appears after beating the Cathedral (rather than Sheol), which is available in the Wrath of the Lamb DLC.

This ending is very confusing at first, but i think i now understand the whole thing:
[WARNING, LENGTHY THEORY AHEAD]

The entire "Binding of Isaac" is just a game Isaac made up in his imagination.
His ideas are scribbled down on papers that he does while his mom watches Christian Broadcast.
She is religious and doesn't play with him much.
He likes monsters, but also lets himself inspire from his surroundings, particularly Christianity-related things through his mom. He keeps his world with scribbles and toys inside his Chest.
She doesn't like the monster-related stuff, hence she keeps taking things away from him if she thinks they are not good for Isaac.
So she maybe never really tried to do anything bad to him (let alone murder him).

Isaac either misinterpreted her intentions or dramatized them because he is annoyed by his strict mom.
So he keeps to himself and plays the way he wants with the stuff in his room.

Every now and then, Isaac decides to read a bit in the Bible to inspire himself for new stuff he could think of.
After reading a couple of stories, he compares them to his mom's actions and comes to the conclusion that while his mom might seem to neglect him and takes her religion too serious at times,
she just wants the best for him. He takes a look into the mirror and realizes that he, on the other hand,
keeps himself occupied with monsters although he knows she doesn't like that, and even depicts his Mom as one.
He is the actual monster that stands in front of him in the mirror.
He remembers the games he played when he looks at his Chest*, mopes, and gets a bad consciece.

*The games he played are shown in the 11th Ending (after beating Satan), where you can se him walk to the Chest in Sheol.
Upon opening it, the round is over for him. His thoughts flickers through every level and every persona he plays in his game.
He finds himself in his room for a short period of time, but realizes this is not where he wants to be.
Instead, he immediately wants to continue living in his imaginary world, and crawls inside his chest to keep on dreaming.
(I call this form of escapism the "Spongebob-Cardboard-Box Principle".)

I might be completely wrong, odds are i over-interpreted a 15 second video :D

EDIT: After yet another update, it turns out that this is not the absolutely final ending either. We got a 13th ending with new information that, again, puts various aspects into a new light.
