Attempts to complete the game at maximum difficulty without taking damage. At maximum difficulty, you can only take two hits, with no means of restoring/increasing health. Several projectile/enemy patterns are also different. The latter half of the game in particular requires pattern memorization to complete, let alone without taking a hit; with only two hit points, there is already minimum room for error.
This is the US version, which has infinite continues and some graphical differences, as well as minor translation differences. A few parts of the health/lives balance is also slightly different between the US and Japanese versions.
Splatterhouse 2 was a Sega Genesis exclusive sequel; there was no arcade version equivalent.
