Simple mistake caused an early death this time around.
Spelunky Lesson of the Day: While killed enemies can trigger black powder boxes with their blood, make sure you are nowhere near them when the blast happens. Those boxes are as possible as bombs, almost always meaning instant death if you're caught in the blast as you're unlikely to have more than 10 HP in the Mines.
