True-BS mode can only be unlocked after completing the blackout challenge which is unlocked by completing frantic first. You can do true-bs mode in either casual or frantic difficulty. What you can immediately notice is there are no rings (unless you collect a shield after holding another shield) or checkpoints, only shields and your skill.
