No ghost is broken. Pull off ghosting is allowed, but hammer on ghosting isn't allowed. The exception being that ONE hammer on ghost under the top note is allowed.
I think a ghost should be counted as any change in the TOP note; that would prevent any extra notes from being played, akin to how one would play a real guitar to avoid hearing any unnecessary notes. It would, however, for example, prevent registering BY(G)R(G) inputs for a BYRG quad, but at least that's realistic, as you wouldn't want to hear the lowest note on a real guitar.
