S1E8: What AuDHD Actually Means: More Than the Sum of Its Parts
If you have been told you have both autism and ADHD, you might have heard it framed as a checklist — here are the autism traits, here are the ADHD traits, and together they add up to your experience. The research says something more complicated and more important than that.
The impairment is not additive. It is synergistic.
Additive would mean two sets of challenges that sum together. Synergistic means the two conditions interact in ways that produce outcomes worse than either would predict alone. The whole is greater than the sum of the parts — but in this case, greater means harder, more complex, and more difficult to treat.
Today's primary paper is by Suen and colleagues, published in 2024 in Psychiatry Research, a peer-reviewed Elsevier journal. The study followed adolescents and young adults across a year, comparing mental health outcomes in those with ASD only, ADHD only, AuDHD, or neither. Participants with co-occurring AuDHD had significantly worse outcomes at the one-year follow-up than those with either condition alone — higher rates of depression, anxiety, and overall psychiatric comorbidity, greater functional impairment, and lower quality of life.
This is not surprising intuitively for those of us living it. But having it documented in the research matters, because it has clinical implications. The executive function challenges of ADHD interact with the cognitive rigidity features of autism in ways that can produce meltdown profiles, regulation failures, and burnout that neither condition alone would predict.
The paper also draws on a concept I find genuinely clarifying — shared vulnerability with differential elaboration. Autism and ADHD arise from overlapping genetic and neurological risk factors, but those risks elaborate differently depending on what other factors are present. For late-diagnosed AuDHD women who received one diagnosis first and are wondering whether the other applies — the research increasingly supports the idea that these are not two separate events. They share roots.
