Maybe it wasn't him, because the timing i'm not sure really adds up... his note over near the tiger room made it seem like he was changing as he wrote that, so if he dropped it there he must've turned there... but the note we found outside the mirror room seemed like a pretty final note too... and it wasn't signed, so it could've been someone else... also, there's the whole thing with Trevor seeming to have changed back in the '60s, i think he'd've rotted away by now, or disposed of since a lot of the notes suggest a new batch of researchers came in during the '80s... but then, it also says Trevor built this place, and he hid in that room when things went to poop...
Y'know what? I'm over-thinking it. It was Trevor. One way or another, yeah, i'm just going to accept it could've been Trevor! Good enough.
