I remove a the head end of a broken cable that was stuck inside a Shimano shift lever (brifter). The cable was broken and frayed. It was also installed wrong which made it harder to get out. It's not a standard problem, so there is not a standard solution for this, so you have to figure it out. Improvise. Adapt. Overcome. Invent a solution.
FYI, I know the surgical clamp is called a hemostat.
