Mafia: The Old Country is a 2025 action-adventure game developed by Hangar 13 and published by 2K. It takes place before the events of the first game and is the fourth main entry in the Mafia series.[1] The game was released for PlayStation 5, Windows, and Xbox Series X/S on August 8, 2025.

Mafia: The Old Country is a third-person linear, narrative-driven game set in cinematic environments.[2] This brings the game more in alignment with Mafia and Mafia II but still maintaining the open-world orientations.[3]

Mafia: The Old Country is set in Sicily, Italy, during the 1900s in the fictional town of San Celeste, which was previously featured in Mafia II.[4] It is based around the origins of the Sicilian Mafia.[1] The narrative follows Enzo Favara (Riccardo Frascari) who journeys from a childhood of indentured labor as a carusu in Sicily's sulfur mines to joining Don Torissi (Johnny Santiago), the head of the Torrisi crime family.[2]
