With motor_stop enabled and min_throttle set to 1150 as I normally fly, the rear left motor was having trouble starting. To fix this:

DISABLE motor_stop
Set arm/disarm to a switch
Set min_throttle to 1200 (I have mine set to 1240 as the problem did not get fixed till then)
