Only happens if you attempt to battle the Elite 4 after defeating your rival in Kanto's Mt. Moon on Monday or Wednesday. It can be done only once on those days. This has always been somewhat "hidden" as an encounter from the player, as you have no reason to go back to the Elite Four once you've defeated them until after you defeat Red. At which point, defeating the Champion again will respawn Red in Mt. Silver, which isn't clear to the player. So many players never encounter this final Silver battle.
