I know a lot of people have trouble with PoFV extra. It's easy to slip up a little and die as Komachi, and if you aren't used to her you're probably going to get hit in the face a few times by spirits. Ugh. A few extra lives would be nice, wouldn't they?
Well, here's what helped make this level go from annoying to fun for me - stop playing defensively. Go for score, go for getting as many bullets on the screen as possible, and try to get to bonus. As soon as you start playing recklessly spamming fairly level attacks, your score will shoot up, especially for the end of round bonuses. Since Score = lives on this stage, it means you will have a huge number of lives to throw away by the ending stages.
This was a particularly BAD run for me, dying twice (really stupidly) to Shiki, twice to Reimu('s balls), once to both Marisa and Yuka, and I end with a life in stock anyway. With my style, even though I die on Yuka, and I end up one life higher than I went in.
Sikieiki is still hard, but if you throw away a few lives at her the attacks become fairly easy to dodge.
