TLDR: leech lasts up to 5 seconds by default, and changing the rate of leech never changes that.
For builds that usually never rely on leech, just slap on wurms and leech flask (nothing on tree required) to recover your full life in 5 sec (20% life/sec) when you take a hit (but you have to be under full life, unless you have a source of leeching at full life).
If you use immortal ambition + forbidden rite every 5 sec, it's possible to perma leech.
