Since flying IFR more, my use of physical checklists has had to become routine. For good reason. There's so much more that can go wrong and should it go wrong, the consequences can be severe. I spent a lot of time writing my checklist and then refining it. In fact, there are new things I know I need to add to it even now.
On my flight back from the Netherlands the other month, I was distracted by a call from ATC which caused what in Human Performance terms is described as a 'lapse', which if not caught, could have caused me big trouble.
