Orbital and periorbital (preseptal) cellulitis are infections involving the tissues around the eye that can appear similar but differ significantly in severity and management. Periorbital cellulitis is confined to the soft tissues anterior to the orbital septum and commonly presents with eyelid swelling, redness, and warmth, usually without visual impairment or painful eye movements. In contrast, orbital cellulitis involves structures within the orbit and is a medical emergency, characterised by pain with eye movement, reduced vision, proptosis, ophthalmoplegia, and systemic features such as fever. Orbital cellulitis most often arises from sinus infection and requires urgent imaging, intravenous antibiotics, and specialist involvement to prevent serious complications including vision loss, intracranial spread, and cavernous sinus thrombosis.
