After editing this vlog, I looked up Ken Kesey to see what the inspiration was for this novel, and apparently he was a nurse's aide at a psychiatric hospital; his time there inspired his views on psychiatric institutions and he based some of his characters on patients he met.

I recommend this NPR article which delves into some of the themes of the book and why Kesey did not care for the film adaptation:
