Sailor Moon's second transformation which appears in the anime episode 51 "A New Transformation! Usagi Powers Up" Usagi held up the Crystal Star as its lid opened, revealing the Silver Crystal. Usagi waved her hand in front of the Crystal, making it emit a pink light, then held it suspended between her hands, above her head. She drew her hands down until the brooch was in front of her torso, where it turned over and affixed itself to her chest. She spun around several times, then wings appeared affixed to her back as pink ribbons formed her leotard. The wings then vanished and more pink ribbons formed her gloves and boots. Her skirt and back bow appeared and a crescent moon began to glow on her forehead; after a moment the moon was hidden by her tiara when it, her odango covers, and her earrings manifested. She then finished in her final pose. Enjoy! :)
