A successful MEMORY.md write proves the file exists. It does not prove every byte entered model context or influenced behavior.
Current Claude Code documentation says the first 200 lines or 25 KB of MEMORY.md load at startup, whichever comes first. Topic files can be read on demand. Keep saved, loaded, and used as separate facts.
