Every single person I know has some kind of medical difficulty. This song is all about wanting to be with someone even if they do have more serious issues and the push-pull that goes along with that, where either party pulls back and doesn't want to disappoint the other party if they don't make it.

For the imagery, I opted to go for a dual problem situation. So imagine someone with type 2 diabetes dating someone with a heart condition. Can still live full long lives, but you never know what will happen since none of us are promised tomorrow.

Depressing, if you look at it from the loss perspective, instead of the "I love you, even though" aspect, which is the vantage here.
