Watch to see the location of the Malfunction skull, the first skull in the prequel bonus mission Boarding Action.
After defeating a group of Covenants and two Hunters, instead of going up to the left to where Sgt. Johnson wants you to go, instead go to the right and down the ramps to go to the lower level. Then go across the walking platform, through green doors, and head a bit right until you see the bridge, which is above the area you just fought through. The skull is in the middle of the bridge.
The Malfunction skull removes a random element from your HUD upon respawning.
