A Tyranid splinter fleet from Hive Fleet Leviathan has launched an invasion of the Imperial-controlled Recidious System. In response, Archmagos Nozick Beta-12 of the Adeptus Mechanicus urgently requests assistance to defend the jungle Death World of Kadaku and to protect a classified project known as "Project Aurora." Veteran Deathwatch Space Marine Demetrian Titus, who has served for nearly a century, is dispatched with his Kill Team to support the defense of Kadaku. During their descent, Titus is separated from his squad and becomes the sole survivor. He successfully launches a virus bomb into the stratosphere in hopes that it will gradually eradicate the Tyranids on the surface. Although he is mortally wounded and surrounded, he is rescued by the Ultramarines.
Upon waking, Titus finds himself aboard the Battle Barge Resilient, where he is greeted by Chaplain Quintus. He learns that, under orders from Chapter Master Marneus Calgar, he has undergone the Rubicon Primaris, a transformative procedure that converts Firstborn Marines into Primaris Marines, who are more formidable than their predecessors. Feeling disgraced and wanting to return to the Deathwatch, Titus is reminded by Quintus that he has been cleared of chaos taint and that true redemption requires him to return to his chapter and fight the Tyranids.
Returning to the Battle Barge, Titus reports Nozick's death and the involvement of Chaos to Acheran. Acheran orders Titus to locate Nozick’s apprentice, Morias Lueze, on the Hive World of Avarax to continue Project Aurora. Titus and his squad locate Lueze at an Adeptus Mechanicus facility. While accessing a cogitator, Titus uncovers a shocking revelation: Project Aurora is powered by a shard of Inquisitor Drogan’s Power Source, recovered by Stenguard Malum Caedo centuries ago on Graia. Titus confronts Acheran with the truth, demanding the project be halted and the Power Source destroyed. However, Acheran refuses, believing that Project Aurora is key to winning the war.
Back on Avarax, Titus and his squad are ambushed by Thousand Sons forces searching for Lueze. After battling through the traitor forces, they successfully escort Lueze to the Battle Barge. During the journey, Lueze reveals to Gadriel and Chairon that Titus was once the Captain of the 2nd Company two centuries ago and was thought dead after destroying the Power Source during the Graia campaign. This revelation deepens Gadriel's suspicion and resentment towards Titus. Titus urges Acheran to contact Chapter Master Calgar before proceeding with Project Aurora, but Acheran dismisses the idea, citing that the Astropathic Relay necessary to contact Calgar is overrun by Tyranids. Titus proposes that he and his squad clear a path to the relay while another Ultramarine squad eliminates the Hive Tyrant leading the Tyranids. Acheran agrees, and Titus sets off on the mission.
After battling their way to the Astropathic Relay and witnessing the Hive Tyrant's defeat by the other squad, Titus meets Astropath Neoma. During her ritual to contact Calgar, Neoma accuses Titus of treachery. Gadriel attacks Titus, and in the chaos, Chairon, suspecting a trap, shoots Neoma. The ensuing explosion of Warp energy allows Sorcerer Lord Imurah to breach real-space. Following a brief confrontation, Imurah escapes, and Titus returns to Acheran with the news.
On the surface, Titus and his squad find a Necron obelisk powered by the stolen Power Source, which has opened a Warp Rift allowing more Chaos forces to arrive. Reuniting with Acheran, Gadriel proposes shutting down the lesser obelisks fueling the main one. The squad battles through a massive force of Thousand Sons, but is saved by the arrival of Marneus Calgar and his reinforcements. Titus and his squad disable the lesser obelisks while Calgar and the Ultramarines distract Imurah's forces. After shutting down the last obelisk, they confront the main obelisk guarded by a Lord of Change.
Imurah lures Calgar into a trap, leading him through a warp portal. Titus follows, and together with the Ultramarines, they rescue Calgar and battle Imurah and the Lord of Change. Titus seizes the Power Source from the distracted Imurah and destroys it, causing Imurah to disintegrate. When Titus awakens, he finds himself in real-space with the victorious Ultramarines. Calgar praises Titus and invites him on a new mission. Titus says farewell to Gadriel and Chairon, and is approached by Chaplain Quintus, who congratulates him but warns of the ever-present risk of corruption. Quintus then removes his helmet to reveal himself as Leandros, Titus' former squadmate who had originally reported him to the Inquisition.
