I play through the entire No Mercy campaign of the Left 4 Dead June 2008 internal prototype build. I noticed quite a lot of little differences here that were changed for the final release 5 months later which i'll include as timestamps below where possible.
00:00 Different title screen and basic source engine options menu. Every time I open a menu it plays the Glimpse_of_Hell_01_01 sound and the title music has a lot of zombie noises not seen in the final release.
01:39 The lobby looks different and you can't actually start it as a single player. I don't know if you can when more people join. I used the console to add bots where I take one of the slots as Louis. It seems to be random who you end up being.
02:10 The intro is different, longer and the blue tarp that blows away stays attached a lot longer here (there is one in the final, honest).
Every safe room makes you wait 30 seconds before you can leave, even if you die and even if you're the only human player.
Obviously the character models are different and the pistol shot sounds are different.
3:40 the vote menu is different and includes the option to ban people from using grenades?
04:17 Meleeing bots makes them jump which can help make them become unstuck in this build.
05:51 Breaking these windows allow you to reach the street much faster which the developers probably didn't intend. The final game has unbreakable metal frames over the windows.
07:00 It's much quieter in general with less talking from the other players, less noise from specials when they're close and less zombie noise and less zombies.
08:19 The hunter makes a woosh noise when he jumps and an odd pain noise when he's on fire.

*on a different playthrough getting knocked back by my own pipebomb caused me to stumble backwards into something and get temporarily stuck.

Also, this ladder isn't here in the final or the hatch you can open from the top. The scissor lift goes back down again in this build.
