A random secret you find close to a key you have to find in the sewer at the third floor of the sewer. (Actually the first floor when you just enter in the sewer zone of the game.)
Don't annoy or hate the creature picture on the wall... it may get a revenge against you.
Actually, when you press use on it, it spawn a couple of those sewer crocodile monster behind you and you are somewhat trapped surrounded by them. (I had God Mode so that's why there was so many noises when they were biting me. In the game with God Mode activated, it makes noises everytime you get hit by something. Sometimes it gets really annoying.)
