Title: The role of continuity of care in moderating the impact of multimorbidity on health care and outcomes

International researchers, physicians and health services and health policy decision makers met at the Alfred Nobel Forum in Stockholm, on 21st May, 2018. Evidence gaps in the fields of research, clinical practice and health policy were discussed and shared among the ca. 120 participants.
