Unfortunately Extreme & Despair loses a turn over other difficulties on Wave 1 since Specials now cost 2 AP and we're forced to use Defensive Stance or Dash with Hiruko on Turn 1 otherwise we Game Over. Wave 2 is still a simple matter of just doing 20 damage to the boss, which Hiruko easily accomplishes. Love her.
