UPDATE: The doll will still spawn even if you get the message, “A ton of blood flowed out.” I’ve seen the doll spawn for those that got this message. Ton of Blood, Torrent of Blood - both seem like region specific. Just make sure to wait a minimum of 15 mins before heading down to the grinder area and not to kill the zombies by yourself.

This video will show you how to solve RE9's Final Puzzle.
To complete the Final Puzzle and get 20,000 CP, you will need to start a brand new game. Once you reach the Processing Pool in the Underground/Basement area, DO NOT turn the handwheel. Save your game and wait for 15 minutes to let 115 bodies fall into the pool. This is needed to get Marie’s Doll, a key item needed to solve the puzzle. After the time has passed, turn the wheel handle and drop down into the area. Once the processing starts, DO NOT kill any of the zombies yourself. You must let the grinder kill all of them. You can shoot them in the leg and push them. Reload your last manual save if you kill any by mistake. Afterwards, flush the pool and you should get the message that *A torrent of blood flowed out.* Once you return to the Care Centre, go flush any toilet 8 times. You do not need to take the Severed Hand with you nor Emily. Marie’s Doll will appear later during the escape sequence in the secret Research Facility. Complete the rest of the game with the doll in Grace's inventory. DO NOT store it in the item box.
In the next playthrough, after escaping the basement, you need to head to the Lead Researcher's Office with Marie's Doll in you inventory and while holding Emily. Approach the safe that held the Star Quartz and enter the following code: Sun, Sun, Star - Sun, Moon, Star - Sun, Moon, Sun - Moon, Star Moon.
You will hear a laughter and the message will appear that the Final Puzzle challenge has been completed, netting you and extra 20,000 CP.
