3: Describe the video and yourself
4: The video link
5: Send it to LGUCommunity@yahoo.com

Make sure that the video is unlisted. If you can't make it unlisted for some reason, upload it to a file share website like dropbox. If you don't want to use those, than upload it public but only if you have to.
