There are two ending items you can unlock by completing Metal Gear Solid.
During the Sniper Wolf encounter, Meryl will be shot and kidnapped. To rescue Meryl, you must survive all rounds of torture that follow. Failing to do so will only see Octacon survive in the story. This ending will reward you with the Stealth Camo.
Resist the torture, and Meryl will survive - giving you the canon ending, and the Bandana item.
To clarify - to get Metal Gear Solid's two ending items, you must resist torture to get the Bandana, or give in to get the Stealth Camo.
By completing the game twice with both endings, it's possible to have both items on a single save file.
