During one of his streams Hexy tried to instantly kill a survivor simply tapping the M1 key because he was using the ultra rare addon "Judith's Tombstone". But something went wrong, and instead of the killing animation, his character simply swung and the survivor got downed. Because of that, he completly lost his mind for a moment...
