To enter the instance you need a spinal blade Backpiece any level and must also be soulbound not account bound so in other words equipped or you have chosen the stats by double clicking it and resulting making it soulbound. When you defeat Scarlet you gain In a Cage Made of Steel and Lunacy Achievement.

Its an easy mission and to get the achievement you need to finish Scarlet without getting hit by the rings marked on the floor as a reddish/orange ring to get the Achievement Nowhere to Hide.
