The Gufoni (Appiani) maneuver is used to treat the lateral or horizontal canal BPPV. It is an alternative to the BBQ roll which can also be used to treat lateral canal BPPV: youtu.be/mwTmM6uF5yA
For the Gufoni maneuver to work correctly, two pieces of information is required… which side to lay on and which way to turn the head. This information can be obtained while laying flat on the back and the turning the head from one side to the other (supine head roll test). First, determine which side triggers a slow or weak nystagmus (or eye twitching). Whichever side causes a weaker nystagmus is the side you will lay down on.
Next, determine which direction the nystagmus is moving. Is it moving towards or away from the ground? The head turn during the Gufoni is in the same direction as the nystagmus.
Two different demonstrations of the Gufoni maneuver are shown in the video illustrating both variations.
• Supine head roll test (0:17)
• Determine side where nystagmus is weak (0:26)
• Determine which way to turn the head (0:38)
• Demo #1: Geotropic left lateral BPPV (0:52)
• Demo #2: Ageotropic left lateral BPPV (1:29)
