1) HMRC Gateway Login - gov.uk/log-in-register-hmrc-online-services/register
2) Check your Adsense Payment information matches on your name and address according to your tax records.
Title, abbreviated first name, or middle name(s) must be the same through all your records.
3) How to fill in the Certificate of Residence request form (and the link to the form):-

4) If you get rejected, delete the previous submissions beforre re-trying. Once you receive the Certificate of Residence, submit the form (photograph of the document in clear .jpg format) in Adsense, to Google Ireland. ie. In Google Adsense, follow the links to fill in the Tax info for Ireland, it will require you to submit the Certificate of Residence.
