Well, technically not a death screen since it happened while starting the night, but it's called a death screen by the community.
Even though I knew about these screens before-hand (I've gotten the Eyeless Bonnie death screen in the first game, and one of the springtrap death screens on the start-up of night 3 in the 3rd game), it's still really unexpected when it happens, and seeing as I've done around 200 or so tries of this mode and hadn't seen it yet, it took me by surprise.
