This is one of these cheap power showers, very popular in the UK as in older houses the hot water storage tank is not pressurized, just gravity fed by a cistern in the attic, therefore the hot water pressure is usually not sufficient for a normal shower head.
These electric showers are pretty simple, basically just a massive heating element (8 to 12kW) , supplied with cold mains water.
Looks a bit scary inside, i personally don't really like the close proximity of mains power and a shower head. But apparently these things are considered to be safe by UK regulations.
