Here is the buff list for physical weapons:
Lord of Blood's Exultation (talisman) - 20% more damage for 20 seconds when blood loss occurs near or on yourself
White Mask - 10% more damage for 20 seconds when blood loss occurs nearby or on yourself
Kindred of Rot's Exultation - 20% more damage for 20 seconds when someone got poisoned including yourself
Mushroom Crown - 10% more damage for 20 seconds when someone got poisoned including yourself
Black Dumpling - 10% more damage 60 seconds when you get madness bar at full and damage yourself. It stacks with bloodboild aromatic and golden vow type buffs.
For all these 5 items above, these buffs stack, and you don't have to equip these equipment after you activate the buff. You can remove them, or switch to something else.
Red-Feathered Branchsword - 20% more damage(Activates when you're at less than 20% of your HP)
Axe Talisman - 10% more damage for charged attacks.
Counter Attack - 30% more damage. What is counter attack? You hit the enemy with a physical thrust attack just before they hit you. No other type of damage would be included in this buff (magic, holy, fire), only physical damage would be applied.
Spear Talisman: 15% more damage for counter attacks. So your damage will be 49.5% more with the counter attack. This type of attacks is very powerful, even if your AR is not as strong as other weapon, you can deal more damage with it. I tested this with Claymore against Giant's Crusher at the same level.
Godrick's Rune: +5 to all attributes
Twinbird Kite Shield - 5% more damage (Activates when you're at less than 20% of your HP, you can two hand your weapon with this. And you don't lose much damage since you can twohand your weapon)
Flask of Wondrous Mix - Spiked Cracked Tear - 15% more damage for charged attacks
Flask of Wondrous Mix - Strength Knot Cracked Tear - +10 Strength...
Rallying Standard ash of war - 20% more damage for 30 seconds (from Commander's Standard weapon)
Bloodboil Aromatic - 30% more damage for 60 seconds (only works for physical attacks, it drops your damage negation though)
Royal Knight's Resolve ash of war - 80% more damage for one hit only, works for 10 seconds (only works for physical attack weapon)
Seppuku Ash War - Use for Blood loss (can be used on sword and katanas)
Fetid Pot - Use for poisoning yourself to activate the Mushroom Crown
Any madness inducing incantation or weapon like Vyke's Spear.
There are also some debuffs that you can apply to enemies or bosses:
Soul Stifler ash of war from Winged Greathorn weapon: Drops the defenses of enemies by 15% who are caught in the area. It lasts 10 seconds though. So you can deal 15% more damage to any enemy.
