Updated video guide for Well-read achievement (I know I'm a few years late).
This updated guide now covers all 13 collectibles. Hector's Demise ones are not required for the achievement.

Important notes: To unlock collectibles, you must finish the Hellraid first. After saving Lucius, you will unlock Raid mode which makes the collectibles available.
Rooms now have RNG. it took me 2 runs to get all 10 collectibles for the Well-read achievement. First run only had 8.
Lord Hector's collectibles are purely for completionists. You do not need them if you're going for the achievement. They don't even count towards the achievement progress.
To get Lord Hector's Expedition memo, you need a specific door to go through. If it's not unlocked simply restart until it is. Shouldn't take longer than 3-4 tries. (Has to be the door on the left when entering the room with skeletons that come through portal.) 05:51
