In low-income countries as many children die following hospital discharge as die during their hospitalization. In Uganda, this means that about one in 20 children die within 6 months of hospital discharge. The Smart Discharges program is leveraging the digital transformation in clinical care throughout Africa to improve post discharge outcomes. With simple risk algorithms that use routinely collected clinical data to identify children who are especially vulnerably to poor post-discharge outcomes. This allows health workers to ensure these children receive improved follow-up care, allowing limited health resources to be used more efficiently while at the same time improving outcomes for the most vulnerable.
