Set in 2014, five years after Metal Gear Solid 2, the world has been transformed by endless proxy wars fought by private military companies. At the center of this new battlefield is Old Snake, a rapidly aging Solid Snake whose body has been genetically engineered to deteriorate at an accelerated rate.
Snake is recruited by his former commander, Roy Campbell, for one final mission: assassinate Liquid Ocelot, the leader of a massive PMC army who is attempting to seize control of the Sons of the Patriots, a global AI-controlled military network known as SOP. With SOP controlling soldiers' weapons, emotions, and combat abilities, Liquid Ocelot plans to overthrow the existing world order and create a system where soldiers can fight without outside control.
Snake's mission takes him across the Middle East, South America, Eastern Europe, and Shadow Moses Island. Along the way, he reunites with old allies including Otacon, Meryl Silverburgh, and Raiden, while encountering the deadly Beauty and the Beast Unit and uncovering the truth behind the Patriots.
As the conflict escalates, Snake discovers that Liquid Ocelot's plan is far more complicated than it appears. The Patriots' system was created by the mysterious AI known as J.D., based on the will of the legendary The Boss. Ocelot intends to destroy the Patriots' control system using a modified version of Metal Gear REX and a stolen nuclear weapon.
Snake eventually learns the shocking truth about his own existence, Big Boss, and the origins of the Patriots. The final confrontation between Snake and Liquid Ocelot becomes a battle not only between two legendary soldiers, but between the old world of war and the possibility of a future without the Patriots' control.
In the game's emotional conclusion, Snake prepares to end his own life, believing his accelerated aging and FOXDIE virus make him a danger to others. However, Big Boss unexpectedly appears and reveals the truth about the Patriots before dying peacefully beside his son. Snake is given a new reason to live, choosing to spend his remaining time peacefully with Otacon and those he loves.
