From the "Star Trek: The Original Series" season 2 episode "Return to Tomorrow" on the "Roddenberry Vault" Blu-ray release when using certain audio tracks.
There is an interesting error that you can hear after the Desilu logo fades out. You hear the end of the Desilu logo audio repeat once before the CBS Television Distribution logo fades in. The AVID Wiki describes this error as "...most likely an editing mistake made while inserting the Desilu logo over the original Paramount logo." However this description is not very accurate. It's simply poor audio editing by the editors by having the remastered audio (which contains the CTD logo audio) cut off the selected audio, without editing out the Desilu logo audio on the remastered audio.

These Star Trek season 1 (and early season 2) episodes originally didnt have any copyright info. Later on, a 1978 copyright notice wouldve been added onto the Desilu logo. For the Blu-ray release of season 1, they plastered the CBS Paramount Television logo with the CBS Television Distribution logo. As a result, a 1978 copyright notice was added to the CBS Television Distribution logo.")
