Companions' Reactions to Being Chosen for Sacrifice (BOOOAL & Fish People in Baldur's Gate 3)

In Baldur’s Gate 3, there’s a secret area in Act 1 that many players might overlook—the Festering Cove in the Underdark. This cave is home to strange amphibian-like creatures called Kuo-toa, and it offers some unique choices that can change the course of your journey. In this video, we’ll show you how to find the Festering Cove and become Mahkloompah, the Kuo-toa's new god.

First, let’s talk about how to get to the Festering Cove. The easiest way is through Auntie Ethel’s lair in the swamp. After defeating her, you’ll find a mushroom portal that leads to a set of cliffs in the Underdark. From there, you’ll come across another portal that takes you to the lower level of the Underdark, near an area filled with explosive orange plants.

Now, be careful here—these plants will blow up if you get too close, so it’s best to use a ranged weapon to clear the path. Once the area is safe, look for a Cragged Rock in one of the dark corners. You can climb down from here to enter the Festering Cove.

In the Cove, you’ll encounter a group of Kuo-toa, led by a figure named Pooldripp the Zealous. The Kuo-toa are in the middle of a ritual, worshipping their so-called god, Booal. Here’s where things get interesting—Booal isn’t really a god, but a creature deceiving the Kuo-toa for its own gain. You can reveal Booal’s true identity by passing a series of skill checks during dialogue.

If you succeed, Booal will attack, but the Kuo-toa, realizing they’ve been duped, will turn on him as well. Once you kill Booal, the Kuo-toa will proclaim you as their new god, Mahkloompah. This is where you get to make some choices. You can command them to build an army, stop their blood rituals, follow your own god, or let them continue as they have. Whatever you choose, Pooldripp assures you that they’ll spread your name violently in your honor.

But if you fail the skill checks, Booal will manipulate the Kuo-toa into fighting alongside him. In this case, you’ll have to defeat both Booal and his worshipers. While you won’t become Mahkloompah in this scenario, you’ll still earn XP and loot, including Booal’s special sickle. However, without the Kuo-toa worshiping him, the sickle won’t grant the same buffs it would if Booal were alive.

Now, there’s another option—if you choose to sacrifice a companion during this event, you’ll earn a powerful buff called Booal’s Benediction. This buff grants advantage on attack rolls against bleeding enemies, but it only works as long as the Kuo-toa continue worshiping Booal. Sacrificing a companion will have major consequences, though, as they can’t be revived and you’ll lose access to their questlines. So, make sure it’s someone you don’t mind parting with.

Overall, becoming Mahkloompah is a fun and unique path in Baldur’s Gate 3. Whether you want to control an army of Kuo-toa, stop their dark rituals, or just gain some XP and loot, the Festering Cove offers plenty of options for how you want to approach the situation.
