Episode #1 Sent in by Scott in Illinois: At an air force base for the B-58 bomber, we have the story from 1964 of how one young aircraft maintainer, by being incompetent and complacent, created a J79 fuel nozzle mismatch disaster. And risked a nuclear disaster... no kidding.

This one is an actual Disaster, instead of the much preferred Near Miss.
we can all learn from this, eh?
