The game features a semi-open world environment and incorporates themes of Lovecraftian and psychological horror into a story that includes elements of investigation and stealth. Despite its title, it is not a direct adaptation of H. P. Lovecraft's short story "The Call of Cthulhu" as it is mostly based on the Cthulhu Mythos along with the 1981 role-playing game of the same title.
