The key lesson is to review industry standards more often. The explosion and fire at the refinery was due to an undetected High Temperature Hydrogen Attack (HTHA) of the steel equipment, which led to the heat exchanger rupture. The CBS concluded that the industry’s standard for determining vulnerability of equipment to HTHA to be inadequate.

In collaboration with CCPS (Center for Chemical Process Safety), this video has been subtitled in Spanish by Pluspetrol with the consent of the Chemical Safety Board, in order to promote the learning of process safety in the Spanish-speaking community.
