Thanks to NCommander for pointing out that I was using the wrong resolution for my CGA display.
Original:
280 RZ(0) = 512:RZ(1) = 390:REM Width & height of screen
Changed to:
280 RZ(0) = 640:RZ(1) = 200:REM Width & height of screen
