Smoke Bomb is an automatic story unlock from Sequence 02 - Mission 01: Lively Havana. Afterward you can select it with L2 + Down / LT + Down - and throw it with L2 + R2 / LT + RT.

This is best done when boarding ships, there are many enemies close together. Alternatively, you can attack enemies in cities or plantations and lure them close together.
