I finally solve the problem!

Occam's Razor once again prevailed.

I also learned something special about this particular generation of opto switches on Stern - they do not use separate transmitter/receiver LEDs... and these LEDs are NOT compatible with other more modern optos. These are some odd, likely obsolete LEDs that can be used as BOTH transmitter AND receiver. That's pretty wild (and potentially clever in terms of BoM provided they're inexpensive, but right now, these LEDs are harder to find and cost more than separate transmitters/receivers - in many cases it's almost as cheap to just by replacement boards than it is to get the individual LEDs and replace them.).
